The version by Ajarn Apichai has to be made to order and constructed through your birthday, and yes, it includes your name and birth details. Please note that the name has been removed on the first photo.

   This Yant is placed in the bottom of an incense bowl under the traditional mix of sand and rice grains used in Thailand. The sand represents the goddess of the Earth, the protector of the land and the world. So when you put incense into the sand it is like putting incense into the earth. Every time you light the incense to do kata, meditate, praise, or make a wish it will bring virtue to yourself and the home. This will help you be heard and known by the angels. The incense can also help you gain samathi through meditation, as well as bring good luck and fortune, and prosperity to yourself and the household. This Yant is also renowned for bringing peace and happiness to the home, and everyone in it.

These Yant plates measure around 2.5" and have to be sent flat. It comes with a calling kata and card of Ajarn Apichai.
